List of Trainers Mary Anzilotti, M.A. Gary Bond, Ph.D.

Anzilotti, Mary, M.A.,
is the Coordinator for the IPRTI and an Area Director for Thresholds,
supervising Housing Options and the Quality Assurance/Insurance
Department. She has a
Master's Degree in Communications from Governors State University,
and an undergraduate degree in science from Indiana University.
She has been with Thresholds since 1990. |

Bond, Gary R., Ph.D., is the Chancellor’s Professor of Psychology at Indiana University Purdue University Indianapolis, where he has been on the faculty since 1983. After receiving his doctorate from the University of Chicago in 1975, he conducted research on self-help groups. From 1979 to 1983 he directed the Research Department at Thresholds. For the past 22 years he has studied effective ways to help people with severe mental illness achieve optimal community integration, with a major focus on two approaches: supported employment and assertive community treatment. In addition to his role as Director of the Illinois Psychiatric Rehabilitation Training Institute, he is directing several funded research studies and is also working with a national group of experts in the development of toolkits to help administrators and practitioners implement evidence-based practices. Cupuro, Rich,
is a prosumer and the Chief Operating Officer of Thresholds
Rehabilitation Industries, the affiliated affirmative business
corporation of Thresholds which provides job opportunities for
Thresholds members (clients). He
is well versed in the reality of running a business and currently
responsible for a budget of three million dollars.

Dincin, Jerry, Ph.D., LCSW, has been the Executive
Director of Thresholds for over 30 years. He has played an important
leadership role locally, nationally and internationally in the mental
health field through his writing, lecturing and as a founding member
of IAPSRS, the International Association for Psychiatric Rehabilitation
Services. Jerry has a Doctorate in Psychology from Northwestern University
and a Master's Degree in Social Work from Case Western Reserve University.

Evans, Jovier, Ph.D., is an Assistant Professor in the Clinical Rehabilitation Program
in Psychology at Indiana University Purdue University Indianapolis
(IUPUI). He has been a member
of the department since 1998 and is an expert in the areas of neuropsychological
assessment, cross-cultural counseling issues, and mental health needs
assessment. He earned his
Doctorate in Clinical Health Psychology from the University of Miami
and is a licensed clinical psychologist and health service provider
in psychology in the state of Indiana.

Fallon, John, B.A.,
is the Coordinator of Demonstration Projects at Thresholds and
directs two specialized teams working to place people from Cook County
Jail back into the community. Typical clients have 50 arrests, 20
psychiatric hospitalizations and decades of homelessness in their
history. John's 20 years of experience in the mental health field
includes providing residential and outreach services to adolescents,
homeless persons, MISA clients, and severely mentally ill adults and children.

Fraser, Ginnie, CRC, LCPC,
is an Assistant
Director of Thresholds. She supervises Thresholds North, the Sandler
Center, the Mothers’ Project and TRI (Thresholds Rehabilitation Industries).
She has a Master’s Degree in Rehabilitation Counseling from
the University of Wisconsin at Madison and has been with the agency
for over 20 years.

Kerouac, Joe, M.Ed., has been the Education
Coordinator for the Thresholds Community Scholar Program (CSP) since
1989. CSP is the agency-wide supported education program for college
students with mental illness. Joe is chairperson of the Thresholds
Education Committee. He is also an Adjunct Instructor in the HealthCare
Leadership Department at National-Louis University in Evanston. He
holds a Bachelor of Science Degree in Elementary Education from Illinois
State University, Master of Education Degree from the University
of Illinois and a Master of Educational Administration Degree
from Governors State University. He is a consumer, consultant and published
author in the fields of supported education and consumers as colleagues.

Kim, Hea-Won, M.S.S.W., Ph.D., is an Assistant
Professor at the School of Social Work, Indiana University.
She received her Doctorate in Social Work from the University
of Wisconsin-Madison. She
has worked on a variety of research projects such as development
of fidelity scales to monitor implementation of psychiatric services
and evaluation of impacts of antipsychotic medications on vocational
outcomes for people with schizophrenia.
In addition, she has done clinical consulting for the Adult
& Child Community Mental Health Center in Indiana.
Her interests center on the role of family in the recovery
process for people with severe mental illness.

Little, Nancy, M.A., is Director of the Training Department at Thresholds and has been
with the agency for over 15 years.
She has a Master’s Degree in Counseling Psychology from
the Adler Institute and has trained on various topics such as MISA
Issues, psychopharmacology, “Train the Trainer,” and diagnosis and
assessment.

Lysaker, Paul H., Ph.D.,
is a clinical psychologist and the Clinical Supervisor for the
Vocational Rehabilitation and Day Hospital sections at the Roudebush
Veterans' Administration Medical Center in Indianapolis.
He is also an Assistant Professor of Psychology in the Department
of Psychiatry at the Indiana University School of Medicine.
He holds a Doctorate in Psychology from Kent State University.
He has over 15 years of experience providing and supervising
mental health services for persons with severe mental illness, including
psychosocial and psychotherapeutic interventions. He also has more
than ten years of experience conducting research on psychosocial
interventions for persons with schizophrenia and has published
extensively in that area.

Mayes, John, M.S.W., LCSW,
is an Assistant Director of Thresholds and is responsible for the
Training Department, Bridge West, Bridge South, Austin Apartments,
Metro West, Thresholds South, Thresholds Southwest and the MISA Rowan
Trees program. He holds a
Master's Degree in Social Work from the University of Illinois and
has been with the agency for over 15 years.
He has extensive experience as a clinician, administrator
and educator.

Nathanson, Fran, M.S.W., LCSW, has been with Thresholds for over 25 years, and is currently the
Outpatient Services Coordinator at Thresholds North.
She received her Bachelor of Arts Degree from Washington
University in St. Louis and her Master of Social Work Degree from
the Jane Addams School of Social Work at the University of Illinois.
She has trained on topics such as medications, borderline
personality disorder, childhood sexual abuse, HIV, and sexual
orientation.

O’Neill, Sheila, LCSW,
is
the Thresholds MISA Coordinator.
Prior to that she was the director of managed care at Thresholds.
She has trained nationally on the impact of managed care
on community mental health.
She has been with Thresholds since 1983, both as a clinician and
as an administrator.

Rupp, Julia, CRC,
is an Assistant Director of Thresholds and an Adjunct Faculty
Member with the Psychology Department at Elmhurst College.
She has presented at local, statewide, and national conferences
and has many years' experience training on a variety of topics,
especially dual diagnosis and vocational rehabilitation.
She has been a surveyor for the Council on Accreditation
of Rehabilitation Facilities (CARF) since 1993.
She holds a master's degree from DePaul University and is
a Certified Rehabilitation Counselor.

Sacks, Yvette,
is a prosumer with extensive experience organizing state conferences.
She has worked in mental health services as a direct service
provider. She served on the
Mental Health Consumer Education consortium (1998-2000), and currently
serves on the planning committee for the Illinois Office of Mental
Health Metro South consumer conference.
She is also an active member of the National Alliance for
the Mentally Ill.

Salyers, Michelle P., Ph.D., is a clinical psychologist and an Assistant Scientist in the Department
of Psychology at Indiana University Purdue University at Indianapolis.
She also serves as Co-Director of the ACT Center of Indiana,
a state funded technical assistance center for assertive community
treatment. She earned her
Doctorate in Clinical Rehabilitation Psychology at IUPUI in 1998.
She completed her clinical internship at Dartmouth Medical
School and was a National Association of State Mental Health Program
Directors post-doctoral fellow at the New Hampshire- Dartmouth Psychiatric
Research Center. She has
been active in mental health services research and has authored or
co-authored several articles and book chapters in the field of psychiatric
rehabilitation, primarily in evaluations of assertive community
treatment and supported employment. Her current research interests
also include trauma and posttraumatic stress disorder among people
with severe mental illness.

Shimon, Susan, M.S.,
is
Executive Director of WilPower, Inc., a psychiatric rehabilitation
and supportive housing agency serving people with severe mental illness
in the Chicago north suburban area.
She is past president of the Illinois Chapter of the
International Association of Psychosocial Rehabilitation Services (IAPSRS)
and a former member of their national board of directors.
She has presented throughout Illinois and nationally on
the topics of consumerism, self-advocacy, elimination of stigma,
and connection to larger community networks of people in recovery.
She also served on the demonstration project team for a
research project sponsored jointly by Indiana University Purdue University
Indianapolis, the New Hampshire-Dartmouth Psychiatric Research Center
and Thresholds, examining the comparison of two employment models
for persons with severe mental illness.
She co- authored the article "A Business Solution to a
Rehabilitation Problem", Psychosocial Rehabilitation
Journal, that was later included in the IAPSRS publication,
An Introduction to Psychiatric
Rehabilitation.

Vogler, Kathleen Ph.D., HSPP,
is
the Chief Psychologist at Larue Carter Memorial Hospital and a Clinical
Assistant Professor with the Department of Psychiatry at the Indiana
University School of Medicine.
She is a licensed, clinical psychologist endorsed as a Health Service
Professional in Psychology (HSPP) by the state of Indiana.
She is also Certified Rehabilitation Counselor.
She earned her Bachelor of Arts in Psychology Degree from
Rosemont College in Rosemont, Pennsylvania.
She has a Master of Science in Psychology Degree with an
emphasis in Rehabilitation Counseling, and a Doctorate in Clinical
Rehabilitation Psychology, both from Purdue University in Indianapolis.
She has previously held various positions in vocational
rehabilitation and counseling.

Ware, Frank H., M.S.W., ACSW,
is the Executive Director of the Janet Wattles Center in Rockford,
Illinois and serves as Adjunct Faculty for the School of Social Work,
University of Iowa. He
earned his Master of Social Work Degree from the University of Iowa
in 1979. He is a past
president of the Community Behavioral Health Association in Illinois
(CBHA) and a former member of the Joint Advisory Council to the Office
of Mental Health. Currently
he serves on the Executive Committee of the Rockford Mayor's Task
Force on Homelessness.

Zahniser, James H., Ph.D., is an Assistant Professor
of Psychology at Greenville College in Greenville, Illinois.
He earned his Doctorate in Clinical and Community Psychology
from the University of Maryland at College Park, and is a licensed
psychologist in Colorado and a licensed clinical psychologist in
Illinois. He was Director
of Outcomes Assessment and, later, of Program Enhancement (evaluation
and training) at the Mental Health Corporation of Denver from 1996-1999.
He has worked with state and local human services providers
to evaluate mental health and psychiatric rehabilitation programs, and
to provide training in consumer-provider partnerships.
He served on the Center for Mental Health Services’ Task
Force on Competencies for Direct Service Staff Members Who Work with
Adults with Severe Mental Illness in Outpatient Public Mental Health/Managed
Care Systems.
